Insurance
Dr. Lowenthal is out of network with all insurance companies. All office visit (virtual and in-person) and case management charges can be submitted to insurance for reimbursement. We will gladly provide superbills with all appropriate insurance codes.
Fees
Initial and One-Time Assessments (can be submitted to insurance for reimbursement)
Includes in-person or virtual review of medical records, detailed history and symptoms, meeting with patient and family members/support system, physical exam, lab testing, EKG and other studies as appropriate.
90 minute comprehensive evaluation - $1500
Follow-Up Appointments (can be submitted to insurance for reimbursement)
In-person and virtual appointments with the patient present.
15 minute appointments - $190
20 minute appointments - $250
30 minute appointments - $375
45 minute appointments - $560
60 minute appointments - $750
Case Management (can be submitted to insurance for reimbursement)
Treatment team/family video meetings, treatment team phone calls, transfers to higher levels of care, form completion etc. are billed at the same rates as above.
Monthly Access Fee (not applicable to one-time consultations)
Unlimited patient and family phone, email and text access to Dr. Lowenthal. Up to two hours of email and text communication with treatment team providers. - $900/month
